General Data Protection Regulation (GDPR)

In line with the General Data Protection Regulation (GDPR), which comes into force on 25 May 2018, I can confirm the following:

That brief Case Notes are taken of our sessions and filed securely.

These notes act as an aide-memoire to support the therapeutic process. The Case Notes will refer to you by code only, not by name. No personal details are recorded in Case Notes.

The paper copy of the original contract is stored in a secure safe, separate from Case Notes.

In the event of an emergency; for example, if I am incapacitated by a serious illness, life crisis (such as bereavement of a close relative) or my death, your contact details will be accessed by the Executors of my Professional Will. The Executors are also counsellors and will contact you to offer support in the interim I am unavailable.

In line with best practice, and following guidance from my Supervisor, I will retain Case Notes for adults for a period of 3 years and for under 18’s 5 years.

You have a right to access your Case Notes at any time.

I am registered as a ‘data controller’ under the Data Protection Act 1998, with the Information Commissioner’s Office (ICO), the UK’s independent body set up to uphold information rights.
